As part of our caring community, you will use your knowledge, skills and professionalism to enhance the quality of life for patients through:
Providing initial comprehensive and ongoing assessments of Palliative Care patients Developing a comprehensive plan to assure palliation of symptoms and improved quality of life Providing visits to patients' homes in various setting wherever the patient calls home Completing patient visit notes and billing records in adherence with regulatory requirements and company policy.
Providing practical, psychosocial and emotional support, resource procurement, and education about advance planning to palliative care patients and their families.
Experience and Education:
Licensed Clinical Social Worker (required) Two years of experience working in palliative care/hospice care or a related field Valid Colorado driver's license, proof of auto insurance and reliable transportation.
